Health Services

We contract with School Health Consultants to:

  • Promote student health
  • Provide disease intervention
  • Counsel student and staff on health
  • Act as a liaison with community health resources

School Health Consultants also organize vision and hearing screening and ensure students meet state immunization requirements. 

Nurses visit schools on a rotating schedule.

Annual Health Information Request

Each year, we ask families to complete a Confidential Health Information Form. The form helps us learn about your child’s allergies and medical needs.

Please give a completed form to your school office.  A nurse will follow up with you if she has questions. ​

Vision and Hearing Screenings

All students in grades K through 5 and 7 are given vision and hearing screenings. Students in 6th, 8th and 9th grades are screened if they meet any of the following criteria:

  • New to the CK District
  • ReceiveSpecial Education services
  • Screen is requested by the parent or teacher

If your child is not scheduled to be screened this year you may request a screening by contacting your child's teacher or by emailing Health Services with your child's name, school, grade and teacher and any concerns you may have.

Parents are informed by mail if their child does not pass screenings. 

A professional evaluation by a physician is recommended for students who do not pass their vision or hearing screenings.  If you have any questions regarding the results of your child’s screening, please contact the school nurse.
